Our Story & Mission
Community Health Providers (CHP) was founded on a simple belief: everyone deserves access to compassionate, high-quality healthcare. We help individuals and families overcome barriers such as cost, lack of insurance, and limited access to care by connecting them with affordable medical services, prevention programs, and trusted healthcare resources.
Our mission is to improve health through early intervention, education, and personalized care while treating every patient with dignity, respect, and compassion.
What We Do
CHP provides comprehensive medical services with a strong focus on prevention and long-term wellness.
Services include primary care, chronic disease management, HIV and STI testing and treatment, PrEP, PEP, DoxyPEP, sexual health services, patient education, care coordination, and community outreach. We also help eligible patients access patient assistance programs, insurance resources, affordable care options, free condoms, and other prevention services.

Many people delay healthcare because of financial hardship, lack of insurance, transportation, language barriers, or difficulty finding a provider. CHP works to make care easier to access before health concerns become more serious. With HIV and STI prevention remaining an important need throughout South Florida, we bring prevention, testing, treatment, and sexual health services together with primary care to help improve individual and community health.
Providers: Join the CHP Network
CHP is expanding its provider network to increase access to HIV/STI prevention, screening, PrEP, PEP, and treatment services throughout Miami-Dade. Participating providers receive program training, clinical support, patient navigation, eligibility and documentation assistance, and linkage-to-care resources — with no upfront cost to participate. Eligible providers may also receive compensation for approved program services in accordance with CHP agreements. Interested in joining? FREE PrEP Pills
Stay ahead of HIV with PrEP, a simple daily pill proven to greatly reduce your risk of infection. Yeztugo is a twice a year injectable to reduce the risk of HIV.
